Identify exactly what the decision says
Read the complete letter, enclosures, cited policy language, definitions, exclusions, conditions, dates, and factual statements. Distinguish a coverage denial from an under-deductible decision, partial denial, estimate disagreement, reservation, or request for information.
Make a numbered list of every reason and locate the supporting policy page or claim record. Save the delivery date and any stated review or response instructions without guessing at legal deadlines.

Compare reasons with the original evidence
Use photographs, timelines, inspections, mitigation logs, inventories, estimates, receipts, and communications to compare each issue. Note what the insurer reviewed and what does not appear in the analysis.
Never change or exaggerate evidence. Seek truthful alternatives for missing records, such as vendor logs, earlier photographs, permits, maintenance records, or proof that a document was submitted.

Ask focused questions in writing

Build an issue matrix for the next review
Create one row for each stated reason and columns for the cited policy language, insurer fact, your supporting record, unanswered question, submission date, and response. This makes it easier to distinguish a cause dispute from an omitted room, valuation question, policy condition, or record the insurer says it did not receive.
Before sending another package, confirm that filenames are clear, pages are legible, and every attachment is tied to a numbered issue. Explain photographs by room, date, orientation, and observable condition; explain estimate differences by component, quantity, measurement, or method rather than only by total price.
